He was predeceased by: his aunts and uncles, Angelo D'Agostino, Dana Maas (Robert) and Jill Forman. He is survived by: his parents, K. C. Forman and Carmella Forman; his brothers, Patrick and William; his grandparents, Joseph, Linda D'Agostino and Marilyn Forman (late Kenneth Charles Sr.); and his aunts, Claudia Leland (Randy), Diane Carstens (Charlie) and Barb McCleave (Jeff).

Plans for a celebration of life for Christopher are pending and will follow a relaxation of Covid restrictions.
