He was predeceased by: his parents, Joseph Cejka and Ludmilla Cejka (Polacek); and his son Raymond (Beverly). He is survived by: his daughter Sandy Fowler (Don) of Geneva; his grandchildren, Patrick Burke, Craig and Paige Fowler; his brothers, Robert and Victor Cejka (Arlene); and his cousin Betty Horacek. He is also survived by nieces, nephews, and friends.

In lieu of flowers, memorials to Pulmonary Fibrosis Foundation, or stopsarcoidosis.org would be appreciated.
