He was predeceased by: his parents, Thelma Kukuk (Murphy) and Merle Kukuk; and his brother Forrest Cain. He is survived by: his children, Kristina Johnson (Luke) of Chillicothe, IL., Suzy Lewis (Mike) of Dahinda, IL., Jeff Kukuk (Amanda) of LaFayette, IN. and Jennifer Kukuk of Schaumburg, IL.. He is also survived by his family and friends.
In lieu of flowers, memorials may be directed to Wounded Warrior Project, St. Jude Children's Hospital, or the Freedom Paw Service Dogs Foundation.
