He was predeceased by: his parents, Joseph Cusimano and Lorraine Cusimano (Syverson); his brother James; his sister-in-law Cindy; and his nephew Gabriel.

He is survived by: his wife Sue; his children, Jiana (Jason), Tony (Dawn), Amy, Mike (Sam), Tom and Angela; his grandchildren, Alexandra, Jake, Gabriel, Jasmine, Kailey, Andrew, Brianna, Kayleigh, Bella, Zax, Bradley, Ashlyn, Aiden, Natalie, Tom, Kendra, Dana, Starla and Miles; and his mother-in-law Yvonne Marcure. He is also survived by his wife, children, and grandkids.

In lieu of flowers, a memorial fund has been established.
