He is survived by: his parents, Lynn Folgate and Sheryl Folgate (Martin); his children, Devin Folgate of Rockford and Tara Folgate of Arlington, VA.; his siblings, Tony Folgate (Peggy) of Freeport and Sheri Klindera of Rochelle; his nieces, Hannah Lagman (Connor) and Katie Folgate; and his nephew Ian Klindera.

A memorial has been established in David's memory.
