She is survived by: her husband Jack McIntosh, Sr.; her daughter Vivian Velez; her granddaughters, Ramina Velez and Jacqueline McIntosh; her great grandchildren, Rashad Cannon, Makayla Sanders and Elijah Sanders; and her siblings, Mildred Donald, Rev. Oscar Sims, Dr. Aridell Slaughter, Dr. John A. Sims (Vicky), Shirley Christian (Charles), Lessie Williams, Rev. James Sims (Jeraline), Georgia P. Sims, Dr. Juanita Sims Doty and Casaundra McCullough. She is also survived by many nieces, nephews, cousins and friends.
