He was loved and cherished by many people including: his parents, Leonard Austin and Lois Polk (Timms); his children, Brannon Polk (Lori) of Evansville, Amy Brandsasse of Evansville and Barry Polk of Evansville, Indiana; his brother Jack Polk (Anna) of Bridgeport, Illinois; his grandchildren, Audra Brandsasse, Kasey Ferguson (Taylor), Benjamin Polk (Ellen), Austin Polk and Olivia Polk; and his great grandchildren, Beau Ferguson and Sage Polk.

Memorial donations may be given to the Moose Charities Moose Heart.
