He is survived by: his wife Cathy; his sons, Alex Banzhaf and Drew Banzhaf; his daughter-in-law Eleanor Lipsky; and his sister Deborah York. He is also survived by his extended family, a wonderful network of friends and his ever-grateful patients and their children.
