With profound sadness, we say goodbye to Heather Ann San Juan of Elmhurst, Illinois, whose vibrant spirit touched the lives of many. She is survived by: her significant other Jim Moskal; her children, Aidan and Caeley San Juan (Jim Moskal); her parents, Sharon Pawlak (Seyller) and David Pawlak; her siblings, Diane (Pawlak), Steve Angelucci and JoAnne Clemans (Pawlak); her niece Jill Angelucci; and her nephews, Torin Angelucci, Brandon Clemans and Trent Clemans.
Visitation was held on Monday, April 7th 2025 at 3:00 PM. A funeral service was held on Monday, April 7th 2025 at 8:00 PM at the Gibbons Funeral Home (134 S York St, Elmhurst, IL 60126).
