He was predeceased by: his grandparents, Dick, Audrey Lewis and Robert Miller. He is survived by: his parents, Jon R. Lewis and Lori Ann Lewis (Miller); his spouse Stacey Baker; his children, Mackenzie Lewis and Braxton Lewis; his siblings, Kylie Rhodes (Duane), Whitney Hess (Jeremy Probst), Courtney Sutter (Chris), Brad Lewis, Dylan Lewis and Audra Lewis; his grandmother Betty Miller; his close friend Jami Sidwell; and the mother of his children Stacey Lewis. He is also survived by nieces and nephews.
