He was predeceased by: his parents, Carl Mahaffey and Pauline Mahaffey (Simmons); his son Brad Mahaffey; and his brother Glenn Mahaffey. He is survived by: his wife Karen Mahaffey of Edgewood; his children, Neil Mahaffey (Denice) of Edgewood, Kayla Legg (Ervin) of Mason and Kendra Kuhlman (John) of Mason; his grandchildren, Kacie of Sadie, JT, Allie, Taylor and Avery; his great grandchildren, Karlee, Gracelyn, Maddelyn, Ashlyn, Kinley and Kohen; and his pet Penny.

Instead of sending flowers, the family would appreciate for memorials to go to Shriners, Effingham Humane Society, or the donor's choice.
