He was predeceased by: his parents, Virgil Avery and Marguerite (Rebeke). He is survived by: his wife Nancy Brownfield; his daughters, Jayne Story (Alec) of Houston, TX, Jananne Brockman (Brad) of Somers, MT and Jaye Crouse (Michael) of Herriman, Utah; and his brother Gail Avery. He is also survived by five grandchildren; and nine great grandchildren.

Memorial donations in Jay's name may be made to St. Jude or South Side Mission.
