He is survived by: his sister Susan Chastain (Dave) of North Port, FL; his nephews, Ben Sparks of East Peoria, Brandon Baker of East Peoria and David Smith, II (Tina) of Dunlap, TN; his great-nephews, David Smith, III of Dunlap, TN and Brantley Smith of Dunlap, TN; and his great-niece Aurora Sparks of East Peoria.
