He was predeceased by: his father Wiley Cook. He is survived by: his siblings, June Minard (Tom) of East Moline and James D. Cook; his mother Alice Cook of Moline; and his nephews, Jason Minard (Morgan) and Jeremy Minard (Ashley). He is also survived by nieces; and 2 great nephews.
