He was predeceased by: his parents, Marcus and Daisy L.; his wife Janet M. Henk; and his sisters, Darlene "Boots" Collins and Elizabeth M. "Betty" Simonini.
He is survived by: his children, Denise Johnson (Tim), Mark Jr. Guth (Sarah) and Deanna Guth; his grandchildren, Ashley Petitti (Richard), Logan Johnson and Brynn Guth; his great grandchildren, Skylar and Alexander; and his sister Beverly "Kay" Petges (late Donald). He is also survived by many nieces, nephews, and friends.
