He was predeceased by: his parents, James D. Cook and Lucille Cook (Lange); and his siblings, Kathleen Reilly, Margaret Wood, Robert and Gerald Cook.
He is survived by: his wife Debbie Arthofer; his children, Rob Cook (Carrie) of Dubuque, IA, Lisa Brace (Ken) of Mokena, IL, Wade (Christina), Matt (Paige), Brandon, Derek, Nolan (Ashlyn), Jordan, Devon, Addison, Mason, Harley and Quinn; his siblings, Patricia Kaesbauer (Bob) of Dubuque, IA and Phillip Cook (Cindy) of Dubuque, IA; and his parents-in-law, Jim Reilly, Diane Cook, Marlene Cook, John Arthofer (Shari), Rhonda Williams (Craig), Pete Arthofer (Julie), Trudy Arthofer, Toby Arthofer (Sharon) and Sue Egloff (Ed). He is also survived by numerous nieces and nephews.
