He was predeceased by: his parents, Earl Joseph Lewis and Hazel Rotha Bennett (Dixon). He is survived by: his siblings, Christopher Lewis of Dupo, IL, Julie Lewis of Dupo, IL, Mary Rash of Lenior, NC, Robert Lewis, Jeffery Lewis of Denver and Donald Lewis. He is also survived by aunts, uncles, cousins, nieces and nephews.
