She was predeceased by: her parents, Kenneth Sizemore and Ruth Sizemore (Stroud); her husband Donald D. Cook; her brother Danny Sizemore; and her sisters-in-law, Donna Sizemore and Emma Sizemore.

She is survived by: her children, Ricky Lee Shaw (Sherrel) of Du Quoin, Peggy Teel of Du Quoin, Donnie Cook (Kelly) of Du Quoin, Rodger Cook (Melissa) of Du Quoin and Tammy Doerr of Du Quoin; her brothers, Lyle Sizemore (Shelby) of Du Quoin and Marvin Sizemore (Cindy) of Du Quoin; and her caregivers, Louise Walker, Shirley Stroud and Wilma Norris. She is also survived by thirteen grandchildren, thirty-two great grandchildren, four great great grandchildren.

Friends may make memorials to Residential Hospice and will be accepted at the funeral home.
