He was predeceased by: his parents, Everett Cook and Pearl Cook (Brayfield); and his siblings, Wilma Napier, Alberta Phillips and Emil Cook.
He is survived by: his wife Patricia Ann Sizemore; and his children, Ricky Lee Shaw (Sherrel) of Du Quoin, Peggy Teel of Du Quoin, Donnie Cook (Kelly) of Du Quoin, Rodger Cook of Du Quoin and Tammy Doerr of Du Quoin. He is also survived by twelve grandchildren, twenty-five great grandchildren, and one great great grandchild.
