He was predeceased by: his parents, Athern L. Choate and Mildred P. Disney Choate; his wife Jane Choate; and his siblings, Nita Sailor, Lonnie Choate, Jack Choate and Robert Choate.
He is survived by: his daughters, Julie Crutcher (John) of Delavan and Stacie Brehm (Dave) of Delavan; his grandchildren, Cody Stewart (Mary Beth), Bethany Choate, Britany Choate, Rei Brehm, Billy Brehm and Kaylie Brehm; his great grandchild Vayda Rushing; and his brother Frank Choate of Indiana.
