She was predeceased by: her father James Edward Sullivan. She is survived by: her mother Iva Dean Sullivan (Williams); her husband Bruce Jacobs; her sons, Ryan Jacobs, Drew Jacobs (Lacie) and Cody Jacobs; her grandchildren, Emma, Jackson and Aiden Jacobs; and her siblings, Ron Sullivan, Terri Morton (Tom), Brenda Moser (Gary), Patty Jones (Gary) and Angie Smith (Greg). She is also survived by many aunts, uncles, cousins, nieces and nephews.
