He was predeceased by: his father Jerry D. Smith; and his daughter Kayla Michelle Smith. He is survived by: his mother Cathy Smith; his sisters, Amy Smith and Carla Smith; his grandchildren, Christian Chandler and Chelsea Chandler; and his close friends, Ronnie, Stephanie and Celeste.

Memorial donations may be given to the family to help care for Jerry's grandchildren, Christian and Chelsea.
