He is survived by: his wife Debora Amos; his children, Amber House (Sevren) and Jeffery Cook; his granddaughter Olivia; and his siblings, Tim Cook, Larry Cook, John Cook, Robert Cook, Tom Cook, Jane Wilson, Peg Barker and Mary Cook. He is also survived by numerous nieces, nephews, and great nieces and nephews.
