He was predeceased by: his parents, Leo Cohen and Eldora Cohen; and his siblings, Mike Cohen and Lisa Weaver. He is survived by: his wife Lois; and his siblings, Steve (Kathy), Greg (Pam) and Cheryl Cotten.
In lieu of flowers, Jack requested donations be made to the South Suburban Humane Society (animal shelter) at 21800 Central Avenue, Matteson, IL, 60443.
