He was predeceased by: his parents, Steve Kochalka and Mary Ann Kochalka (Titra). He is survived by: his wife Carol (Derkovitz); his children, Theresa Cummings, Laura Fennell (Derek) and Steven G. Kochalka (Carmen Montenegro); his grandchildren, Katelyn, Isabelle, Josephine, Andrew, Seth and Paulino; and his sister Sue Flavin. He is also survived by numerous nieces, nephews and cousins.

Visitation was held on Monday, December 16th 2024 from 10:00 AM to 11:00 AM at the Ss Cyril & Methodius Cemetery (2001 Maple Rd, Joliet, IL 60432). A Mass of Christian burial was held on Monday, December 16th 2024 at 11:00 AM at the S. S. Cyril & Methodius Cemetery Chapel (2001 Maple Rd, Joliet, IL 60432).
