With heavy hearts, we announce the death of Philip M. Blaze (Countryside, Illinois), born in Western Springs, Illinois, who passed away on August 29, 2018. Leave a sympathy message to the family on the memorial page of Philip M. Blaze to pay them a last tribute.

He was predeceased by: his parents, Philip Blazevich and Marion Blazevich; his sisters, Joyce Bluder and Joanna Blazevich; and his sibling-in-law Paul Kaempf.

He is survived by: his spouse Judy E. Blaze; his children, Kimberly Joseph (Jason), Keith Blaze and Amy Becker (Terry); his grandchildren, Zachary Joseph, Emily Joseph and Allie Becker; his sibling Marilyn Greenwell; and his siblings-in-law, Robert Kaempf (Carole) and Larry Kaempf (Beverly).

In lieu of flowers, memorials may be made to American Cancer Society or Amita St. Thomas Hospice.
