He is survived by: his former wife Earline May Cook (Joyce); his wife Barbara Cook (Allan); his daughter Sheryl Cook of Edwardsville; his grandchildren, Jessica Donaldson of Edwardsville, Jada and Kaydon Donaldson; his sisters, Jo Marie Boyd of Edwardsville and Nancy Cook of Edwardsville; and his close friend Paul Cann of Alhambra. He is also survived by several nieces; nephews; other extended family and friends.

Memorials may be made to the American Heart Association and may be accepted at the funeral home.
