He was predeceased by: his parents, Gerald T. Heavens and Leona M. Heavens (Henke). He is survived by: his wife Donna; his children, Craig Heavens (Debbie) of Plainfield and Julie Jensen (Kent) of Dwight; his grandchildren, Megan, Drew Jensen, Hailey, Jared Heavens, Matt, Katie and Karli Ginnetti; his sister Patti Testa of Tempe, Arizona; his uncle Jack Heavens; his brother-in-law Lynn Gagliardo of Coal City; and his nieces and nephews, Shawn Testa, Jennifer Swincicki (Ryan), Tony Gagliardo (Trisha) and Jenny Russum (Kyle). He is also survived by several great nieces and great nephews, as well as cousins and friends.
