He was predeceased by: his parents, William J. Mayfield and Kathryn Mayfield (Young). He is survived by: his wife Nancy Mayfield of Clinton, IL; his daughters, Dawn Collins of Decatur, IL, Jennifer Crandall (Eric) of Parker, CO, Jessica Layton of Indianapolis, IN and Amanda Mayfield of Chicago, IL; and his sisters, Diana Kay Peifer of Bloomington, IL and Jacqueline Jesse (Jim) of Flannagan, IL.. He is also survived by eight grandchildren, two great-grandchildren.
