She was predeceased by: her parents, Charles A. Provin and June F. Provin (Thomas); and her siblings, Bruce, Ron Provin and Karen Provin.

She is survived by: her children, Keith W. Smith (Kathy), Clinton, Terri A. Davis, Georgia and Steven R. Smith (Marsha). She is also survived by 8 Grandchildren, 12 Great-Grandchildren, and 2 Step-Great-Grandchildren.

Memorials may be directed to Kindred Hospice, Springfield, IL.
