He is survived by: his children, Justin Polak and Adrian Van Deursen; his siblings, Christine Polak-Carroll, James Polak (Nancy), Carolyn Borner (Wally), Catherine Hejmanowski (John), Janine Booker (Anthony) and Adored; his spouse Maegan; and his child Played.
