He is survived by: his parent Janet Fortag; his grandmother Elva Mueller; his siblings, Tammy Borozovich, Davalynn Booth and Rob Pursley; his aunts, Andrea Means and Pat Neff; his uncle David Collinsworth Jr.; his nieces and nephews, Tara Kennedy, Tonya Kennedy, Heather Jolliff, Hillary Booth, Travis Kennedy and Anthony Jolliff; his cousins, Tia Tarlton, Ricky Means, Brian Means, Nichole Setzakorn and Joey Collinsworth; his great-nieces, Addison Sanders, Carsen Griffith, Emma Abernathy, Haylee Roper and Barden Lowery (Clay Kennedy); and his close friend Shawn Kuhn.
