He was predeceased by: his brother Matthew Wade McIntosh; and his grandfathers, Gerald E. McIntosh and Glen Osborne. He is survived by: his parents, Ricky Lee McIntosh of Dix and Gale McIntosh (Osborne) of Alma; his wife Kaley Rae McTall; his children, Rylee McIntosh (Alaina McIntosh) of Centralia and Levi McIntosh of Irvington; his sisters, Stephani Roohani (Babak) of Augusta, GA and Alyssa Carroll (Darren) of Salem; his in-laws, Doug and Donna McTall of Irvington; his sister-in-law Lacey Burmester (Tyler) of Phoenix, AZ; his nephew Joseph; his niece Ruby Roohani of Augusta, GA; and his grandmother Annabelle McIntosh (Hicks) of Waltonville. He is also survived by many aunts, uncles, and cousins.
