He was predeceased by: his parents, Phillip Maddox and Wilmouth Maddox (Schmidt). He is survived by: his children, Jeff Maddox (Beth) of Las Vegas, NV and Cyndi Dawkins (Don) of Effingham, IL; his sister Phyllis Logan of Centralia; his grandchildren, Christina Maddox, Michael Maddox, Allison Gimbert (Sean) and Brendan Dawkins; and his nephews, Kim Logan (Lana) and Rodney Logan (Pam).
In lieu of flowers, memorial donations can be made to the First Baptist Church and will be accepted at Sutherland-Garnier Funeral Home at 235 North Sycamore Street in Centralia, Illinois 62801 (618) 532-3523, who is honored to be of service to the Maddox family.
