He is survived by: his mother Phyllis Hawkins (Murdach); his spouse Kim DeBose; his children, Nicole (Will), Dusty (Tyler), Amber (Kaleb) and Drew; and his siblings, Tom (Angie) and Beth. He is also survived by his 5 grandchildren, many in-laws and several nieces, nephews, aunts and uncles.
