She was predeceased by: her parents, Dallas Erwin and Nellie Erwin (Farmer); her husband Howard W. Churchwell; and her brother Gary Erwin. She is survived by: her sons, Tim Churchwell (Christy) of Crossville and Tony Churchwell (Stacy) of Carmi; and her siblings, Vickie Moore (Tom) of Carmi and Eddie Erwin (Roetta) of Carmi. She is also survived by several nieces and nephews.
In lieu of flowers, contributions in Janet's memory may be made to Coins for Cancer or Pets at Risk and will be accepted at Campbell Funeral Home in Carmi, who are in charge of arrangements.
