He was predeceased by: his parents, William Blazier and Vonda Lee Blazier (Richardson); his wife Phyllis Sue Phillips; and his brothers, Danny Blazier and William Dale Blazier.
He is survived by: his children, Donnie Blazier (Debbie) of Carmi, IL. and Devin Blazier (Ann) of St. Louis, MO; his grandchildren, Megan Stevens (Scott) and Kailey Blazier; his great grandchild Sophia Stevens; his brothers, Darrell Blazier of Carmi, IL, Gary Blazier (Cindy) of Carmi, IL. and David (Rachel); and his close friend Carolyn Linder of Carmi, IL. He is also survived by several nieces and nephews.
In lieu of flowers, contributions may be made in Donald's memory to the Linda E. White Hospice House, and will be accepted at Campbell Funeral Home in Carmi.
