He was predeceased by: his parents, David Carl and Bessie Marie Simmons (Page); and his sister Tina Marie Ridings. He is survived by: his wife Kristie of Carmi; his sons, David W. Simmons, Jr. (Gena) of Carmi and Daryl Simmons (Susan) of Marion, IL; his stepson Gage Williams of Carmi; his granddaughters, Kadence Simmons and Lindsay Simmons; and his siblings, Sandra Kay Mick of Crossville, Cheryl Stanley of Waverly, TN, Michael Simmons of Carmi, James Richard of Carmi, Pee Wee of Carmi, Simmons Howard of Carmi and Scott Howard of Carmi.
