He is survived by: his wife Monique (Gagne); his children, Michelle English (Greg), Julie and John Cook, III (Ashlee); his grandchildren, Reese, Savannah Cook, Aviston, Griffin, Hudson, Leah English, Wren and Loy Cook IV; his great grandchild Eze; and his brother Jimmy Cook (Sandy) of Loveland, Colorado..
