He was predeceased by: his grandparents, Willard, Evelyn Perry and Billie Chadwell; and his uncles, Willard Alan Perry and Joe Beckley.
He is survived by: his father Michael Chadwell (Teresita Perry); his wife Teresita Salgado; his mother Lisa Perry-Frazier (James); his children, Isabella J. Perry and Jesse M. Perry; his stepchildren, Ryan J. Zambrano and Hannah-Leah M. Zambrano; and his siblings, Joshua Perry (Sara) of Ironwood, MI, Patrick Adolphson (Christopher) of Rodgers, AK and Lindsey Adolphson (John). He is also survived by many nieces and nephews.
