With heavy hearts, we announce the death of Jean M. Kozak (Calumet City, Illinois), who passed away on October 17, 2019 at the age of 91. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her husband John J. Kozak; her brothers, Arthur Ludwicki (late Mildred) and Stanley Ludwicki; and her parents, Joseph Ludwicki and Janina Ludwicki.
She is survived by: her children, Cindy Rick (Roy) and Wayne Kozak (Janet); and her grandsons, Brian Kozak (Meryl) and Kevin Kozak (Rachel).
Memorial contributions may be given to Our Lady of Knock Church.
