She was predeceased by: her husband Richard Kovach; and her parents, Joseph Mazur and Pauline Mazur. She is survived by: her daughter Karyn Ballard; her grandson Jeremy Ballard (Samantha); and her brother Jerry Mazur (Mary Jo). She is also survived by several nieces, nephews, great nieces, great nephews and cousins.
