She was predeceased by: her husband John C.'Jack'Phillips; her parents, Thomas C. Pavlick and Barbara B. Pavlick; and her parents-in-law, Jack and Bernadette Phillips.
She is survived by: her children, Melissa and John Phillips; her grandchildren, Bianca (Nathan), John, Joshua, Justice, Jeremyiah and Josiah; her great grandchild Aria; and her siblings, Josephine Derro (Pat), Stephen Pavlick (Jie), Thomas Pavlick (Dorothy), Edward Pavlick (Carol Vaci), Diane Capiak (late Anthony) and Ronald Pavlick (Lisa).
