He is survived by: his sons, Nathan John, Chase David and Shane Werner Roth; the parent of his children Lorel Roth (Simon); his close friend Bob Robinson; his siblings, Kim Gau (Jim), Cindy Gilligan (John), Jim Roth (Lynn Moffa), Bill Roth (Judy) and Bob Roth (Anne); and his pets, Coco, Roxy, Piper and Mike.
In lieu of flowers the family is asking for memorials in Dave's name for a college fund established for his sons.
