He is survived by: his mother Patricia Hazzard (Higman) of Bradley; his wife Jackie Hazzard of Bradley; his children, Caitlyn Hazzard of Bradley and Zack Hazzard of Bradley; his siblings, Julie Robinson of Bourbonnais, Brad Senesac (Geri) of Aroma Park and Heidi Przybyla (Andy) of Bradley; his stepmother Kim Hazzard of Bourbonnais; his parents-in-law, Perry and Debbie Burton of Bradley; and his brother-in-law PJ Burton (Jennifer) of Bourbonnais. He is also survived by many nieces, nephews, aunts, uncles, and cousins.
