He is survived by: his wife Sandra Cook; his sons, Braden and Bryce of Bolingbrook, Illinois.; his mother Linda Cook of Bolingbrook; his brother Jason Cook (Kathleen) of Concord; his siblings-in-law, Louie Amador (Annjel) of Pulaski, Laura Conniff (Edward) of Austin, Deborah Ruiz (Freddie) of Sunray, Brenda Fiest of Tulsa and Bridget Shavers (Curtis) of Bixby; his nieces, Monika Flores (Richard) and Elizabeth Cook; and his nephew Kyle. He is also survived by nieces, nephews and cousins.
