She was predeceased by: her husband Joseph Chlebanowski Sr.; her parents, Frank Lesniak and Frances Lesniak; her siblings, Joseph, Louis, Frank Jr., John, Stanley and Rose; her son Richard (Kathleen); and her granddaughter Emily.

She is survived by: her children, Joseph (Pat), Daniel (Linda), Mary (Bill Perkins), Michael (Carla), Ronald (Norma), Sharon (Shawn Cutting) and Cynthia (Randy Shain); and her grandchildren, Jennifer, Joey, Billy, Rebecca, Ericka, Alison, Stephanie, Natalie, Sara, Rachel, Amy, Tommy, Joshua and Melanie.
