He was predeceased by: his mother Joy Worthey (Lease); and his grandparents, Donald "Wayne", Donna Worthey, Herbert and Bernice Lease.
He is survived by: his father Douglas Worthey (Angela); his stepmother Lisa Edlund (Scott); his sister Tabitha Worthey (Gerald); and his nephews, Keenan Van Note and Evan Van Note.
