He is survived by: his wife Marian Forman; his children, Kim Sweeney (Todd) and Michelle Gooding (Paul); his grandchildren, Samantha Rank (Josh), Morgan Sweeney, Autumn, Ryan Gooding and Blaire Isla Rank; and his brother Gary. He is also survived by many nieces and nephews.
