He was predeceased by: his son Gary Polak; his parents, Frank Polak and Emily Polak; and his sisters, Emily Polak and Betty Schulz (late Glenn).
He is survived by: his wife Meta "Mickey" Polak; his children, Karen Harder (Andy), Beverly Evans (Paul) and Kimberly Borchardt (Tom); his grandchildren, Alyssa Glimco (Chris), Ryan Harder (Thalee), Lucas Harder, Andrew Evans, Michael Evans, Kennedy Setter (John), Christian Borchardt, Grace Borchardt and Aaron Borchardt; and his siblings-in-law, Lorelei Lesak (late George) and Theodore Quednau (Marion).
